Functional Frenuloplasty Case Series

ON DEMAND
Curated by Dr. Soroush Zaghi, MD

This course features 30+ real-world Functional Frenuloplasty cases personally curated by Dr. Soroush Zaghi to highlight clinical decision-making, anatomical nuance, and surgical technique across a wide range of patient presentations.

Each case walks you from pre-operative assessment through surgical execution and post-operative considerations, with emphasis on anatomy, tissue handling, complication management, and functional outcomes.

Designed for clinicians who want to refine judgment, not just technique. Learn at your own pace and return to cases as often as needed to build confidence in diagnosis, treatment planning, and execution.

Deep Dive Course Offerings

After completing this course, participants will be able to:
  1. Differentiate mucosal restriction from fascial and muscular restriction
  2. Identify when a release should remain superficial versus when deeper fascial dissection is required, using intraoperative visual cues and functional assessment.
  3. Apply case-specific surgical decision-making
  4. Adjust technique based on age, anatomy, airway context, behavioral factors, prior surgery, and patient tolerance, rather than using a one-size-fits-all approach.
  5. Demonstrate improved anatomical recognition of the floor of mouth
  6. Identify blood vessels, fascia, muscle layers, and high-risk zones to improve safety, precision, and hemostasis during frenuloplasty.
  7. Select and execute appropriate surgical techniques
  8. Compare and apply laser versus scissor techniques, blunt dissection strategies, suturing methods, glue use, and tension management based on case demands.
  9. Manage complex and revision cases with greater confidence
  10. Recognize scar tissue patterns, compensatory mechanics, and common pitfalls in revision frenuloplasty and difficult cases.
  11. Integrate airway, sleep, and functional considerations into surgical planning
  12. Understand how tongue mobility, posture, UARS, OSA, and compensatory patterns influence surgical goals and outcomes.
  13. Anticipate and manage intraoperative challenges
  14. Including vasovagal syncope, prominent blood vessels, pediatric behavioral issues, and anesthesia-related considerations.
The Deep Dive includes a wide spectrum of cases, such as:
  • Pediatric frenuloplasty under general anesthesia with nasal intubation
  • Functional frenuloplasty in pre-MMA airway patients
  • Fascial-dominant restrictions requiring advanced release
  • Scar tissue and revision cases
  • Management of prominent blood vessels and hemostasis
  • Combined lingual and labial releases
  • Pediatric laser cases with behavioral management
  • Integration of CBCT imaging and tongue posture analysis
  • UARS and sleep-disordered breathing cases
  • And much more!
Each case is annotated to highlight the “why” behind each surgical decision, not just the “how.”

(These themes are consistently reflected across the case examples shown on pages 2–19 of the attached document)
This course is designed for clinicians who:
  • Are transitioning from basic releases to functional frenuloplasty
  • Already perform frenuloplasty and want to refine outcomes
  • Want deeper anatomical understanding, not shortcuts
  • Manage airway, sleep, and myofunctional cases
  • See complex or revision cases and want better pattern recognition
Appropriate for ENT surgeons, dentists, oral surgeons, and airway-focused clinicians working within their scope and training.
  • On-demand, self-paced learning
  • 30+ full surgical case walkthroughs
  • Pause, rewind, and rewatch anytime
  • Designed for repeated viewing as skills evolve
